Immunization records are free to pick up in the office, no appointment needed!

Please bring a valid photo ID with you when coming to obtain a copy of your child's immunization records.

 

VISITS/VACCINES

The American Academy of Pediatrics recommends the following schedule for visits/vaccines.
If you miss or are late with one, you do not have to start over but just resume the schedule.  However, we do not recommend delaying any of them.

IN THE HOSPITAL - HEPATITIS B

DAY 2-5 -          VISIT

2 WEEKS -        VISIT

2 MONTHS -    VISIT AND VACCINES - PENTACEL (COMBINATION OF DTAP, POLIO, HIB), HEP B, PNEUMOCOCCAL, ROTATEQ (ROTAVIRUS)

4 MONTHS -    VISIT AND VACCINES - PENTACEL (COMBINATION OF DTAP, POLIO, HIB), HEP B (IF NOT GIVEN AT HOSPITAL), PNEUMOCOCCAL, ROTATEQ (ROTAVIRUS)

6 MONTHS -    VISIT AND VACCINES - PENTACEL (COMBINATION OF DTAP, POLIO, HIB), PNEUMOCOCCAL, ROTATEQ (ROTAVIRUS)

9 MONTHS -    VISIT AND VACCINES - HEP B, HEMOGLOBIN (TO CHECK FOR ANEMIA)

12 MONTHS -   VISIT AND VACCINES - MMR, VARICELLA, PNEUMOCOCCAL, EYE SCREEN

15 MONTHS -   VISIT AND VACCINES - PENTACEL (COMBINATION OF DTAP, POLIO, HIB)

18 MONTHS -   VISIT AND VACCINES - HEPATITIS A

2 YEARS -         VISIT AND VACCINES - HEPATITIS A, EYE SCREEN

2.5 YEARS -      VISIT AND VACCINES - CATCH UP SHOTS

3 YEARS -         VISIT AND VACCINES - CATCH UP SHOTS, EYE SCREEN

4 - 6 YEARS -    VISIT AND VACCINES - QUADRACEL (COMBINATION OF DTAP AND POLIO), PROQUAD (COMBINATION MMR AND VARICELLA)

7 - 9 YEARS -    VISIT

10 – 18 YEARS - VISIT AND VACCINES - TDAP BOOSTER, HPV SERIES, MENINGOCOCCAL C SERIES, MENINGOCOCCAL B SERIES.

 EVERY FALL, ALL FAMILY MEMBERS SHOULD HAVE A FLU VACCINE AT 6 MONTHS AND OLDER.
