To Request a Well Care Appointment…

At this time, well care appointments are the only type of appointments we can accept requests for from the patient portal.  We hope to add additional visit types in the future.  

If you would like to request a WELL CARE appointment, please select “New Appointment.”

** Completing this form does not schedule an actual appointment, but places a request for one.  You will be contacted by our staff once an appointment has been scheduled.  If we cannot accommodate either of your preferences you will receive a message via your preferred choice of contact to please contact our office.**

Please select “1-Valencia Pediatric Associates” from the Facility List.  It is the first one listed.

Then select the provider you would like to request for the appointment. 

•  If you do not have a preference you may select “No Preference.”

Please remember that the provider you request may not be scheduled to work on the day you request.  We will make every effort to accommodate your requests.

Select the date range you would like to request an appointment for.

• Please note that we do not schedule more than two months in advance.  If you submit a request outside of this range, or if the schedule is unavailable, you will be asked to resubmit a request at a later date.  We will not “hold” an appointment or create a waiting list.

After you have selected a date range, please select your preference for which day of the week you would like.

Please then select your preferred times. We will attempt to accommodate these preferences to the best of our ability.  

Select your method of contact. 

• The information we have on file will be pre-filled in the “Email” and “Contact Number” fields.
• You may change the information on this request to suit your needs.

Please enter a reason for the visit. i.e. 2 Year Physical, Sports Physical, etc  

• Remember, you may only book a WELL CARE appointment via the portal at this time.

Any additional information may be entered into the message field.

*This request will be routed to a receptionist for scheduling.  You will receive a response within 24 to 48 business hours of your request.
